  8. Visits patients/residents identified to be at nutritional risk and family to complete a diet history and periodically to evaluate the excellence of meals served, likes and dislikes.
  9. Participates in discharge planning, development and implementation of patient care plans as required and within designated time frames.
  10. Ensures that doctor’s orders and changes are processed promptly.
  11. Plans and reviews the therapeutic and regular diet plans and menus and ensures that they are in compliance with physician orders.
  12. Provides education to other departments in the healthcare center regarding nutritional services.
  13. Attends and participates in workshops, seminars and in-services to keep abreast of current changes in the health care field, as well as to maintain a professional status.
  14. Reviews complaints and grievances of a nutritional origin and provides verbal/written responses to the NHA/CEO or designee.
  15. Meets with administration, medical staff, nursing staff, and other departments in planning food service programs and activities.
  16. Completes all documentation within the acceptable standards of the department, as well as regulatory bodies including but not limited to Department of Health, Joint Commission or Accreditation of Healthcare Organizations (JCAHO) and the Centers for Medicare and Medicaid Services (CMS). This includes evaluations, progress notes, and plans of care, charges and other necessary documentation.
  17. Collaborates with physicians, nursing staff, ancillary departments and others as needed to provide the highest quality of care to our patients and resident. Assures smooth transitions for patients between levels of care by completing discharge documentation and making recommendations regarding medical nutritional therapy provisions.
  18. Involves the patient/resident and family in planning objectives and goals for the patients.
  19. Demonstrates the knowledge and skills necessary to provide care, and/or interpret information relative and appropriate to the age specific needs of the individuals served.
  20. Other duties as assigned. 

 Qualifications 

  1. Must possess a Bachelor’s Degree in Dietetics from an accredited college or university and must be a registered dietitian or registration eligible by the American Dietetic Association.
  2. Must be licensed in accordance with state regulations in states where licensure is required.
  3. Some long term care/skilled nursing facility experience preferred, and a desire to work with the geriatric population.

Possess a high school diploma or equivalent

Preferably be Med Aide certified, though we are willing to train an experienced caregiver

Have experience working as a caregiver, assisting residents with ADL's, such as dressing, bathing, transferring, and escorting to meals

Be willing to obtain CPR/First Aid Certification

Have a genuine interest in caring for elderly residents and working in a geriatric environment

Possess a willingness to continue learning and improving skills, as well as a commitment to confidentiality

Be able to maintain patient and tactful composure when dealing with residents, family members, staff members and visitors
