For SNF and AL Staff: COVIDVaccination Clinics will be held at a hospital near you (Avera, Sanford, Monument, Mobridge or Prairie Lakes). Your center will be contacted when LTC staff will be scheduled for vaccinations at these clinics
For SNF and AL Residents: Your center previously chose a COVID vaccination provider on the NHSN or REDCap website. Vaccination providers which are CVS, Walgreens and Avera LTC Pharmacy are currently reaching out to each SNF and AL. If you have not heard from your vaccination provider, reach out to your vaccination provider or email Tim.Heath@state.sd.us
