Mille E. Olson Award

Applicants for the “Millie E. Olson Award” must be an employee at a SDHCA or SDALA Member facility that you feel goes the “extra mile.” This award recognizes exceptional service in the long term care profession. SDHCA would like to express to these winners how much we, as a community, appreciate their efforts on behalf of the long term care residents. Only one employee permitted per facility per month. SDHCA will showcase a winner each month in the SDHCA LTC Insider and on the SDHCA website. The nomination form, along with a photo, must be submitted by the 15th of the month. In addition to completing the form below please submit your photo of the nominee to LuAnnSeverson@sdhca.org.
